If you’re just extending the modem some distance from the original phone jack, then all you need is the red and green (if you had a regular RJ11 phone cord) and they are in the middle 2 positions (it doesn’t mater which is which). If you’re using cat 5 to extend the jack, it doesn’t really matter which ones you pick, as long as the same 2 are in the middle. So let’s say you use the blue/blue-white pair. Just make those 2 your middle (position 4 and 5) wires. The others can go anywhere.
Whoever wired up your jack originally decided to use the green/green-white pair in the station cable. Same concept. It doesn’t matter which ones you use as long as the middle 2 are the same.
The positions do matter if you’re wiring it for Ethernet so make sure you clarify if that’s what you’re looking to do.
